Long-term Success and Prognosis
According to Americal Dental Association many failures and complications occur when using implants to support restorations. Most failures can be prevented with proper treatment planning.
Dental Implant failures can be largely classified into four main categories:
- Loss of integration, osseointegration
- Positional failures
- Soft tissue defects
- Biomechanical failures
Failure of the Implant Post to Fully Integrate
For implant treatment to be successful, the implant post that is inserted into the jawbone must fully integrate with the surrounding bone. It’s necessary for new bone cells to grow on and around the post which will help to stabilize it, ensuring it is held firmly in place. This process, called osseointegration can take several months to complete.
Dental implant posts are often specially treated to encourage this process, but like everything, there is always a chance that the implant post could fail to fully integrate, leaving it unable to properly support a replacement tooth and ultimately increasing the risk of failure.
Damage to Nearby Anatomic Structures
Your jawbone doesn’t just consist solely of bone, as it also contains other vital structures that include nerves, blood vessels, and sinus cavities. There is a potential that the placement of the implant could damage some of the structures. Sometimes if a dental implant is placed too close to a nerve it could damage it, resulting in numbness or even pain that may be temporary or permanent.
Damage to your sinus cavities may occur if you have dental implants to replace your upper back teeth. If there is insufficient room to place the implants in this area, then there is a risk the sinus cavity could become perforated. One problem with restoring upper back teeth is that the bone in this area is often naturally thinner because of the sinus cavities which are large air-filled cavities.
This is why people requiring dental implants in this area of their mouth will often need a sinus lift or sinus augmentation, a procedure to build up the bone in this area to ensure this cannot happen.
Bone Loss
Bone loss around the dental implant can affect its stability and it’s something that will require treatment as soon as possible. Sometimes it may be necessary to remove the dental implant and to place a bone graft, replacing the implant at a later date.
If you have bone loss in between your dental implants and natural teeth, then this can create dark triangles that would have previously been filled with gum tissue. These can affect the appearance of your dental implant supported teeth and may make it more difficult to keep your dental implants thoroughly clean.
Infection
Infection is a risk that may occur soon after implant placement or even years later when treatment seemed to have been a complete success. Quite often an implant dentist will give provide a prescription for antibiotics to take prior to implant treatment, to help the implant integrate with the surrounding bone more successfully, but these antibiotics may not necessarily reduce the risk of infection.
When infection occurs years later, it may be due to poor oral hygiene so if you do have dental implants, it’s essential that you remain committed to looking after them through maintaining a good daily oral care regime combined with ongoing professional dental care that includes regular checkups and hygiene treatments.
Implant Fracture or Breakage
It is quite rare for implants to fracture, especially if your implant dentist has used implant components from a reputable implant manufacturer. An implant might fracture if it comes under excessive stress, for example if you grind or clench your teeth, or if the implant has been improperly placed. S
