I am a dentist in Mansfield and I get this question a lot. Amalgam is an alloy of mercury with any other metal. Dental Amalgam is a combination of a silver alloy powders with liquid mercury. Silver-Mercury Amalgam is an old tooth cavity filling material. In the past 200 years, dental Amalgam has been the principal and the most popular restoration material among dentists. It is low cost, easy to form or manipulate, long lasting and it has a good resistance to abrasion. 

Due to its durability and strength, Amalgam has been the best choice of filling material for decades. It has had a proven track of record and it has been an effective filling material successfully used billions of times in human teeth. 

 

Amalgam has been particularly useful in restoring large cavities at the back of the mouth where biting forces are strong. Before mixing, Amalgam is in liquid form. When Amalgam is mixed, it hardens, expands and fills the cavity space inside the tooth. In addition to its long-lasting properties. Amalgam also hardens soon enough. This property, makes Amalgam a good choice of tooth restoration material for treatments with limited available time. Examples are:

Some dentists recommend their patients to change their amalgam fillings with composite fillings (white fillings). Frequent exposure of dental clinic staff to mercury vapour has been recognized as an occupational hazard (possibly when used carelessly). There has been reports that MS patients have benefited from amalgam removal. 

Over the past few decades, there has been an increased number of claims against use of dental Amalgam.  I work near the suburb of Mansfield in Queensland and I have noticed that use of dental Amalgam between Mansfield Dentists is in obvious decline. An indication of an undeniable rising concern among both dentists and patients. Since 2015 I haven’t even purchased Amalgam in my clinic. Dental Amalgam is no longer widely used. Although some health organisations like ADA are still insisting that Amalgam is safe and Amalgam-Toxicity is a scam. 

Risk of toxicity is not the only reason people prefer white fillings over silver fillings. My own sister never accepted use of Amalgam for her fillings. For aesthetic reasons, she wanted and insisted on using a white filling material, the same colour as her teeth not a dark silver colour. She wanted white filling and she wanted “a pretty white filling”. 

Notable disadvantages of Amalgam include:

  • Different from the tooth colour and aesthetically undesirable in the front teeth
  • Conducts temperatures faster than natural tooth 

The ignition of concerns about a possible toxicity of Amalgam drives some patients to request their dentists to change their Amalgam fillings with other filling materials. Patients strongly prefer the more aesthetic white fillings over Amalgam. This demand has changed the tooth preparation and filling procedures. 

Modern Dental Fillings

These days modern filling materials are available in all dental clinics. Composite resin fillings also called white fillings have replaced the old and dark silver fillings. A broken front tooth when restored correctly is hard to distinguish from other neighbouring teeth. Many patients are quite happy about the new technology. Unlike the early generations of composite fillings, the new filling materials have tooth-like qualities. They are not permanent fillings but they are natural looking and their durability is also close to a natural tooth. 

Bottom line is that the best dental filling material for your teeth is no filling at all. Maintain a good oral hygiene and regularly visit your dentist to prevent cavities as much as you can. Gingival grafting has been a common periodontal  practice in dentistry   since the early to mid to 1970’s in the USA.  The method present method used today was foremost propagated by Dr Preston Miller, a pioneering periodontist  in Tennessee.

 

The gingival  grafts that Dr Miller used and is still using have been those of a sub-epithelial connective tissue graft. This is where a U- shaped incision is made in the patient’s pallette, the epithelial layer is then removed and the underlying connective tissue is excised for the purpose of the gingival graft

 

This has been a very effective technique that has been used for years. However recent developments in using skin graft connective tissue from tissue banks has paved the way for the use of an oral skin graft called Alloderm. This has provided an additional way to cover denuded root surfaces in the mouth and has provided an extremely effective alternative to the more conventional method of using grafted connective tissue from the patients pallette. Alloderm ( AlloDerm Regenerative Tissue Matrix)   is a freeze dried skin product, that is readily available and is extremely effective in reconstituting the gingival tissue. It is used for the following:

 

  • Covering exposed a denuded Roots

  • Gingival augmentation

  • Soft tissue ridge augmentation

  • Soft tissue augmentation around the dental implant area.

Alloderm is now widely used throughout the US due to the fact that it supports soft tissue regeneration. It also allows quick revascularization (  the restoration of the blood circulation of an organ or area)  and supports the migration of white blood cells and cell population allowing it to act as host tissue for a strong and natural repair.

 

The use of Alloderm has removed the need to remove the very complicated process of removing sub-epithelial connective tissue from the patients pallette, an often uncomfortable and painful process.

 

Stem cell technology has recently become available to the majority of Americans through the process of using the patient’s stem cells and reconstituting tissues within that patient’s body. We have the ability to take blood from the patient and using special technology separate out the patient’s  maximal platelets with 2 oz of their own blood.

 

When we  combine this with Alloderm, which is the freeze dried skin graft, the patients will then rec alloderm  which is not then and the Alloderm will be infused with their own platelets and stem cells, plateletes  are the cells that are necessary for healing and stem cells will accelerate the healing response because they will be applied alloderm and then placed surgically into the patient’s mouth.

 

We have used this procedure in the past with extremely successful results. There are a number of advantages to using freeze dried skin grafts like Alloderm and infusing it with the patient’s own platelets and stem cells.

Periodontal Reconstruction - Dr. Victor Terranova discusses the  restoration of structure and function of the diseased periodontium.

 

Dr. Victor Terranova received his BA from Rutgers College of Rutgers University in New Jersey. He received his DMD from the University of Medicine and Dentistry of New Jersey in 1972 and his MS and PhD in Molecular Biology at the University of Rochester in 1978. Specialty training in Periodontolgy was completed at the Eastman Dental Center of Strong Memorial Hospital, affiliated with the University of Rochester under the mentor of Dr. Helmut A. Zander in 1976.
After his training Dr. Terranova spent seven years as Senior Staff at the National Institutes of Health and National Institute of Dental Research. Dr. Terranova is currently on the faculty of the California Implant institute. In 30 years of practice, Dr. Terranova has performed thousands of surgical procedures, including implant placement, periodontal surgery and grafts, sinus augmentation, and simple and complicated tooth extractions including 3rd molar extractions.

Periodontal disease was first treated back in the 1950’s and has undergone significant changes in the modality of treatment in the last 4-5 years.  During the 1950’s the main focus of treating periodontal disease was to eliminate  the ‘pyorrhea’ that was found in the patient’s mouth. Pyorrhea was the ‘man in the street’’ term for bleeding, pussy gums.

 

Since then various treatments have been proposed for treating adult periodontal disease and are used effectively. These include scaling and root planing and pocket elimination which has been the very foundation of treating periodontal disease. Getting rid of the periodontal pocket and making the tooth surface more accessible for the patient to clean.

 

Within the last 20 years however the methodology has changed. Instead of eliminating the periodontal pocket by cutting the gum tissue away, people have been more focused on rebuilding the periodontal apparatus in the mouth than the previous method of removing diseased gum tissue and  enlarging the tooth surfaces.

 

By this we mean periodontal reconstructive surgery - rebuilding  the patient’s periodontium ( the specialized tissues that both surround and support the teeth). This entails surgically opening the gums to expose the diseased root surfaces and the bone, then cleaning the inside of the soft tissue flap and reconstructing the ‘boney’ environment around the diseased teeth.

 

In doing this we are undertaking a process now termed ‘periodontal regeneration’. Periodontal regeneration  has been shown to a limited degree. We are now proposing to rebuild the bone structure around the teeth and then consequently place the gum tissue at it’s natural position in the mouth therefore re-establishing a periodontal situation that is disease free and appears natural.

 

By doing this we take bone graft and membrane for guided tissue regeneration and we supplement this with Platelete Rich Plasma  derived from the  patient in conjunction with  the patient’s own mesenchymal stem cells. PRP technology is a highly effective technique used in bone and soft tissue regeneration that yields quicker results and also reduces the discomfort due to improved and faster healing.

 

This process of using PRP and Stem Cells allows for:

Using bone graft material from patient or from tissue bank


In this way the patient’s entire periodontal well being  is rebuilt as we slowly  supply the patient with a disease free periodontium.
